Case Management provides clients one-on-one comprehensive assistance in order to increase their access to healthcare, their awareness of services in the community for which they may be eligible, and help them overcome barriers to achieve economic self‐sufficiency. Share Our Selves’ case management program identifies the individual needs of a patient and coordinates care and comprehensive, easily accessible resources to assist low‐income individuals and families through crisis situations to help achieve long‐range stability.
We also have a Multi-Disciplinary Team (MDT) that takes on the most complex cases that meet a significant level of acuity within our agency. The team consists of a medical provider, a senior case manager, a licensed clinical social worker, a registered nurse, and a public health nurse all working closely to identify and break down the barriers to care.
