With our new secure patient portal, you can save time by using online tools that will help you better manage your health and make more informed decisions about your care – 24/7 from any computer, smartphone or tablet.

You also have the ability to:

  • view test and lab results
  • request appointments
  • request prescription refills
  • send and receive messages from your doctor’s office
  • receive reminders for preventative and follow-up care

Frequently asked questions:

How do I sign up for SOS’ patient portal?

If you are an established SOS patient and are interested in accessing the Patient Portal you can:

  • Click here to be redirected to our FollowMyHealth page where you will be asked to enter your email address, first and last name, and date of birth.
  • Once this information is verified, SOS staff will send you an invitation to the email address you provided. From there you will:
    1. 1. Click the registration link in the email you receive from noreply@followmyhealth.com
    2. 2. Create your portal login using a unique username and password
    3. 3. Enter the invitation code provided by us and click “Agree” to the release of information
  • Choose a Login Method
    You can create a username and password specifically for your portal account by clicking the FMH, Facebook, Gmail, or Yahoo icons.

What are the password requirements?

Password should be at least 8 characters in length, and include at least one numeric and one special character, such as: !@@#$%^)&*-(.

Why has my account been locked?

As a general security feature, the patient portal will lock-out any account that has had (3) failed login attempts. If your account has been locked, contact FollowMyHealth support at 949.270.2196 or fmhpatientportal@shareourselves.org.

I am still having problems connecting with the Patient Portal. Now what?

While most of our patients have experienced no difficulties, from time to time there are technical questions. For technical problems or questions contact FollowMyHealth support at 949.270.2196 or fmhpatientportal@shareourselves.org.

What is the order of providers listed in My Providers?

Providers that are the most frequently visited are located at the top of the list, the least visited provider will be at the bottom.

Is there an app for the iPhone or Android?

Yes, just download the FollowMyHealth Mobile app from iTunes or Google Play.

How far back is my personal medical record available?

You can view your medical records going back to your very first visit at SOS.

Can I ask an emergency related question via the Patient Portal?

No, the Patient Portal is intended for non-emergency healthcare issues only. Please contact us at 949.270.2100 for questions that you would like answered within 24 business hours.

If you are experiencing a medical emergency, call 911 or visit the nearest emergency room immediately.

How secure is my medical information?

Your personal health information is very secure. The FollowMyHealth Patient Portal is HIPAA-compliant in that it adheres to mandated encryption standards when receiving, sending, and storing a patient’s health information. Information shown within the Patient Portal is merely a snapshot of your most recent medical records, not the actual medical record as a whole.

When I check the portal inbox, there are messages from my provider, but I'm not getting any email communication telling me to check the portal. What's wrong?

It could be that your email address on file within the Patient Portal is not documented correctly. Please call us, Share Our Selves at 949.270.2196, and verify the correct email address. Unfortunately, we have no way of knowing if the email reminders and updates from the Patient Portal are getting through to your email inbox. Also, be sure to check your email accounts Spam folder. Some email clients use strict filters and communications from the Portal can be misinterpreted as spam and placed in this folder. If this is the case, denote this communication as “Not Spam”.

Proxy Accounts

Proxy or Dependent Accounts allows you to securely communicate with a patient’s physician office on behalf of a family member or person under your care. This communication is done using an SOS Patient Portal account. For pediatric patients, we will extend Proxy invitations to our SOS Patient Portal to parents/legal guardians named on SOS Children and Family Health Center’s Pediatric Registration Form.

Pediatric Proxy/Dependent Accounts

These types of accounts allow the parent caregiver or guardian to securely access information on behalf of a pediatric patient under the age of 12 years old. On the patient’s 12th birthday it will automatically terminate due to privacy concerns.

Adult Proxy Accounts

These types of accounts allows access to help manage adult patient care who is 18 years of age or older. In order to obtain access, the patient can invite another person, or complete the FollowMyHealth Proxy Account Authorization form allowing the Adult Proxy permission to obtain access. These steps are required to ensure that we allow access to people whom you want to have access to your personal medical records online; it is necessary for security purposes.

Sharing your Portal Information with a spouse or other caregiver.

A patient can also grant proxy access to their own SOS Patient Portal account to another person.

  1. 1. Log into your account and select Settings > Change Account Access.
  2. 2. Enter the email address of the individual you would like to grant proxy access to.
  3. 3. Select a pass code for the individual to enter when accessing your account.
  4. 4. Provide this code to the proxy.
